Note: Teachers will be notified via email of the students they serve in which you may need to review a specific link. Your signature on this form verifies your having completed both policy and link reviews. Based upon the Individual Health Plans of the students you serve, one or more of the following links may need to be viewed:

The school nurse will notify staff of the Individual Health Plans they are required to review by the end of September. Notification will include information on how to access via moodle.
I attended a meeting in which the above policies were brought to my attention. I have reviewed each of the policies on the MAPS website or I have requested a hard copy and reviewed. I have also reviewed or have been made aware of the required Blood-Borne Pathogen Exposure Control Plan link, other required health condition links and the Individual Health Plans of the students I was directed to review.
