Athletic Forms:  Students are encouraged to fill out the Athletic Forms for Students and Fall Sport Sign-up using Skyward Student Access.  Parent forms will now be located in the on-line registration for 2024-2025 school year.

NEW THIS YEAR: Parochial/Home-based school student forms will be in Skyward. If you need sign-in credentials, please contact Sarah Fuchs at 715-536-4581, ext. 10007.

ALL Athletic Forms need to be submitted before the 1st practice or students will not be able to participate. Please don’t wait to schedule physicals. More information will be given at each Parent-Athletic-Coach (PAC) Meeting.

Fall Sports Sign-ups will start July 2, 2024.
